If You Used Your VA Benefits at Another Institution

- Select form 22-1995, Request
For Change of Program or Place of Training (3 pages)
- Complete form 22-1995 and fax to (616) 732-1150. NOTE: You must list
the Davenport University address below as your new school (9A):

Campus location – (Location of campus student will attend)
- You will receive a Letter of Eligibility from the Veterans Administration
- Davenport University Certifying Officials will certify your enrollment
one month before the start of the semester. Certifications consist of the
number of credit hours in which the student is currently enrolled by start
and end dates of each session and semester.
- You must verify your enrollment once a month via the Web
Automated Verification of Enrollment (WAVE) on the VA website.
- The VA will direct-deposit your check or mail it to you.
- It is your responsibility to make any additional payment arrangements on
your tuition bill.
