Student Access Forms
A student will need to complete the following steps in order to begin the process for accommodations at Davenport University.
The Accommodation Request link should be completed by the student. The Verification of Disability Form (VOD) must be completed by a medical professional and can be uploaded with the Accommodation Request form or submitted to your Student Access Coordinator.
